What is Palliative Care?
Palliative Care is a specialized home-based, in-person medical care for people living with serious illnesses, such as cancer, dementia, renal disease, and congestive heart failure (CHF)/cardiac conditions.
Our specially-trained team ~ including doctors, nurse practitioners, nurses, social workers, and other specialists ~ works closely with your current doctors to provide an extra layer of support. Each discipline comes together to care for the whole person and support their circle of caregivers.
The goal of Palliative Care is to improve the quality of life for both you and your family. It can be provided alongside of curative treatment, including home health services.
Our practitioners will come to your home to provide medical assessments, offer routine care to best manage chronic conditions. Our services offer greater convenience and a more comfortable, personalized experience for you, particularly when mobility issues or limited access to transportation make office visits difficult.
