Connecting with community health resources

We can help connect you to local resources that may be available to assist with necessities such as meals, caregiver support, social support and other services you are finding difficult to access because of your illness. We will also provide education on cultivating your best quality of life while living with illness.

Advance Directive Planning

Planning ahead can ease worries for those living with chronic and complex medical conditions. Advanced health care planning helps you make decisions now about the care you’d want if you became too ill to speak for yourself. Thinking through what matters most to you, where you find meaning in your life, who you would want to make decisions for you and the care you may want or not want in the future are all a part of advanced care planning. We will walk you through the steps to develop a plan you feel comfortable with now, so that you don’t have to worry if any health changes come about.

Health Care Advocacy

Navigating our healthcare system can sometimes feel complicated. We can actively support your efforts in getting the quality care you deserve and require due to your illness. We can help you understand your options when it comes to the care you receive. We can also assist you in finding providers, as well as any expertise you may need for insurance or financial decisions.

Home Safety Planning

Home safety planning can assist you with maintaining your independence by reducing risks of falls and potential home accidents that may send you to seek emergent medical supports. We can help by assessing for safety needs to allow you to set up your home in a way that will keep you safe and comfortable. Our goal is to help you in maintaining your goals of staying in your home for as long as possible.

How To Refer

Referrals – Community

Anyone can refer a patient to Snowline. If you know a person who is seriously ill and needs support, or you know someone who you think may be ready for ElderCare, call Snowline at:

Snowline will arrange an initial evaluation. If Snowline determines that the patient is hospice eligible, Snowline will assist in obtaining a physician’s orders to begin comfort care.

 

Referrals – Medical Professionals

If your patient is seriously ill and needs Snowline ElderCare in-home services, please call 530-621-7820 or 916-817-2338.
To refer a patient for Snowline ElderCare services, please complete the Request for ElderCare Orders and fax the completed form with supporting documentation to the Referral Office at Snowline:
